Preparing for Your First Appointment

Self-Paying Patients

You can schedule an appointment directly with us. To finalize your appointment, payment is required prior to your appointment.

Once your appointment is confirmed, you will receive an email to register for our patient portal as well as a text message with a link to complete the patient intake and consent forms. Before your first appointment, please complete your intake and consent forms. These forms are required for us to see you. 

Insurance Paying Patients (Cigna, Aetna, UnitedHealthcare, Oscar, and Oxford)

You can schedule an appointment directly with us. However, please note that your appointment is not confirmed until your insurance is verified. 

To confirm your appointment, we'll need to verify your insurance. Our insurance verification service (either Alma or Headway, depending on your coverage) will contact you by email to begin this process.  Your appointment is confirmed once your insurance is verified. If verification isn't complete before your scheduled appointment, we'll work with you to reschedule.
